12/30/2023 EnchantaVaughn Approved QMHPA greeted Ms. Brown upon arrival to the home setting. Upon arrival to the home setting, Ms. Brown appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Ms. Brown verbalized to QMHPA that her morning was going well. QMHPA explained to Ms. Brown that this session was going to focus on ways to manage her frustration and assist with increasing her social skills. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Brown current thoughts, feelings and emotions. QMHPA engaged Ms. Brown in a skills building activity. QMHPA informed Ms. Brown that the purpose of this activity was to demonstrate appropriate calming techniques when experiencing depression through role play. QMHPA explained to Ms. Brown about being able to endure in enjoyable activities, group sessions and hobbies. QMHPA explained to Ms. Brown the difference from relaxation and just lying in the bed when she is bored. QMHPA encouraged Ms. Brown to participate in more community activities such as the one that she has been participating in at the Calhoun Community Center to increase her social skills. QMHPA discussed with Ms. Brown presenting positive behaviors throughout the community by communicating with people through effective communication skills. QMHPA explained to Ms. Brown how isolative behaviors can cause self-destructing behaviors. QMHPA identified with Ms. Brown inappropriate ways to manage depression. Ms. Brown appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Brown was able to perform some of the exercises that was demonstrated. During the skills building activity, Ms. Brown verbalized to QMHPA that she will have an attitude with others and she doesn’t like to be bothered by others when she is depressed. Ms. Brown verbalized to QMHPA during the session that she enjoys walking. Ms. Brown stated she does relax but has things on her mind or just lay in the bed because she is tired due to her chemotherapy. She indicated to QMHPA at times, she wishes she didn’t have a mental issue because she doesn’t like taking medications because she is already being drained by what she has to endure weekly as well as other stressors that come along with that when it comes to her hair falling out and her having to cut her hair several times to prevent it from shedding and she doesn’t like being depressed or moody all the time.
